What information is collected?
There are 3 main types of data which Curzons Speech Therapy retains:
1. Clinical Data needed to provide a service
Written or spoken information may be collected about your child and immediate family members. This includes name, date of birth, address, telephone numbers, email addresses, developmental, medical and birth history. This information is used to inform diagnosis and intervention planning.
2. Financial Data from clients for billing
3. Contact Data from referrals
Information may be collected from other professionals working with your child, such as NHS Speech and Language Therapists, education staff, childminders etc. Permission will be obtained in advance.
Lawful basis:
The lawful basis for Curzons Speech Therapy, collecting and storing personal data falls under the “Legitimate interest” category of article 6 of GDPR.
Your child can not be assessed, diagnosed and treated appropriately without processing personal data.
Health data falls under “special category data”, section 9 GDPR.
Healthcare professionals are legally required to keep patient information confidential, it is under this condition that data is stored and managed by Curzons Speech Therapy.
How is this information used?
Information obtained is used to inform and deliver speech and language interventions and reports.
Information is used to contact you via text, phone, email and post about upcoming appointments, general communications surrounding appointments, sending reports, therapy management plans and resources and sending invoices.
Communication preferences will be discussed with you where applicable.
How is this information stored?
Curzons Speech Therapy keeps both physical and electronic records of clinical data in order to provide a service.
All electronic information is stored on password protected devices.
Hand written information such as assessment forms are stored in a locked filing cabinet.
Where video or audio data is collected a separate consent form will be completed, this data will be collected on a password protected device and will be kept for 24 hours following the appointment, and it will then be deleted.
No personal data will be stored in cloud storage.
Clinical data is deleted/ confidentially destroyed after 7 years from last invoiced session or until your child reaches 25 whichever is sooner in line with Health and Care Professions Council (HCPC) recommendations.
Financial Data is kept for 6 years to adhere to revenue guidelines.
Financial Data (including non-payment of bills) can be given to Inland Revenue at their request.
Contact Data is kept for 6 years to allow processing of Financial Data if required. This may be retained for longer to for safety, legal request, or child protection reasons.
If you require your clinical data deleted or removed, you must send an email to

Financial data cannot be removed before 6 years; however, it can be shared with you to allow for any correction or for you to make a health insurance claim.
Contact data cannot be removed before 6 years, however it can be shared with you to allow for any update or correction of material facts such as spelling of names, addresses, phone numbers etc.
If you require data to be shared with any third party, you must request it via email to the therapist, or via written request. Only the person listed as the key contact can request this if it is in the case of a minor.
Any information prepared which has not been paid for cannot be shared.
How to access records?
Curzons Therapy must comply with the provisions of section 2(1)(c) of the Data Protection Acts 1988 and 2003. The Acts set out the principle that personal data shall not be kept for longer than is necessary for the purpose or purposes for which it was obtained.
Individuals have the right to ask an organisation if they are using their personal information.
It is within your right to request a copy of yours/your child’s data records as outlined in GDPR. This is known as a subject access request (SAR).
All subject access requests (SAR)’s are to be submitted to kate@curzonsspeechtherapy.com with the heading “subject access request and the date”. Records will aim to be sent within 7 days from the request being received. Please be advised the legal turnaround for this process is one month.
The therapist, Kate Curzons is registered with the Information Commissioners Office (ICO) as a data controller.
Curzons Speech Therapy is committed to effective records management retention and disposal to ensure that it adheres to legal standards in terms of retention periods, optimises the use of space, minimises the cost of record retention and securely destroys outdated records.
